Services we offer for your patients
Medical Care
If you are referring your patient to us for primary care or specialty services to be established as a new patient, please fill out the form below and indicate this in the “Reason for Referral” section of the form. More information on services offered at our clinic can be found here. Your patient will be contacted to schedule their new patient appointment.
B-12 Shots
We offer methylcobalamin or hydroxocobalamin intramuscular injections. We can provide monthly shots, but if your patient requires more frequent shots, please fax us a prescription to the fax number at the bottom of this page. Your patient will be contacted to schedule their B-12 shot. Please inform your patient there is a $30 charge not billable to insurance for this service.
Phlebotomy
We can draw for most LabCorp, Quest, & speciality labs. Please have your patient bring in the printed requisition, any lab kits and their insurance card. Your patient will be contacted to schedule their blood draw. Please inform your patient there is a $30-40 charge for all blood draws that is not billable to insurance and the fee is variable depending on the complexity of the draw.
FibroScan
We can complete in-office FibroScan and share a PDF of the results with you. This procedure is billable to our in-network insurance plans (Regence, Premera, Likewise, First Choice) or the patient can elect to pay a $144 cash price. Simply fax a referral for the Fibroscan procedure with the patients' contact information, insurance information, and diagnosis codes, or use the form below
